Legalizing immigrants for medical reasons and obtaining acces to health care may be obstacle-ridden for both the patient and the physicians involved. The aim of this work was to optimize the actions of the physician in issuing requests for access to care and a prompt le- galization of his or her patients. The present work consisted of a field survey, a collection of statistical data concerning the legalization procedure both in Isère and all of France, and a review of the medico-legal literature. The interviews targeted the needs of physicians and revealed malfunctions in accordance with existing literature. In Isère, on average during 1998-2009, patients seeking legalization are predominantly male (64%) African (84% total, 52% Algerians) suffering from psychiatric disorders (36%). The fraction of positive responses was 65%. The number of applications decreased by 50% in 2008. In France, patients are mostly African (75%) and HIV positive (18%). Barriers for acces to health care and legalization are mainly administrative and illegal, and result in an increased duration of the patients precarious situation, worsening their condition. This delay in legalization has negative conse- quences on individual and collective health. We propose tools in the form of three leaflets to inform doctors on the procedures. They have been submitted to the authorities for validation, and physicians have expressed interest in accessing them online. The outlines of letters, medical certificates and reports could be downloadable and adaptable for each patient, optimizing the medical side of the procedure, taking into account practical constraints. ; La régularisation médicale et l'accès à une couverture maladie peuvent ressembler à un parcours du combattant, tant pour le malade sans papiers que pour le corps médical. L'objectif de ce travail est d'optimiser les démarches du médecin pour entreprendre des demandes d'accès aux soins et de régularisation, en vue d'une entrée rapide de son patient dans le droit commun. Le travail a consisté en une enquête de terrain, un recueil des données statistiques iséroises et nationales et une revue de la littérature médico-juridique. Les entretiens ont ciblé les besoins des interlocuteurs et relevé des dysfonctionnements superposables aux données de la littérature. En Isère, en moyenne entre 1998-2009, les patients sont majoritairement des hommes (64%) africains (84% au total, 52% algériens) souffrant de pathologies psychiatriques (36%). Le taux d'avis positifs est de 65%. Le nombre de demandes a chuté de 50% en 2008. En France, les patients sont majoritairement africains (75%) séropositifs (18%). Les obstacles aux soins et à la régularisation, principalement administratifs et illégaux, entraînent un allongement de la durée de précarité des patients, aggravant leur pathologie. Ce retard d'entrée dans le droit commun a des conséquences néfastes sur la santé individuelle et collective. Nous avons réalisé un outil pratique sous forme de trois dépliants en cours de validation pour orienter les médecins, qui souhaiteraient, à terme, un accès en ligne à ces outils proposés. Les trames de courriers, certificats et rapports médicaux type pourraient être téléchargeables et adaptables pour chaque patient, optimisant le côté médical de la procédure, prenant en compte les contraintes de terrain.

La Belgique est souvent déclinée en deux voire trois chiffres : Flandres – (Bruxelles) – Wallonie et devient ainsi une réalité tridimensionnelle largement répandue par les média ou le politique. Ce numéro deRegards économiquesvise à montrer que l'opposition Nord-Sud n'est pas aussi simple et immédiate : chaque région possède un territoire au sein duquel se déploient les activités qui y sont entreprises, qu'elles soient de nature sociale, économique ou culturelle. En analysant plus finement les disparités à l'intérieur de chaque territoire, on aboutit à une lecture plus subtile du territoire belge. En mobilisant différentes cartes, la première partie de cet article offre une image simple, mais suggestive, de la structure géographique de l'économie belge. La deuxième partie a pour objectif de présenter les éléments théoriques pouvant expliquer l'image qui se dégage de nos cartes. Dans la dernière partie, nous quittons le terrain purement scientifique pour proposer quelques pistes de réflexion portant sur le futur de la Belgique. Bien que très liminaire, notre analyse suffit pour mettre en lumière quelques faits qui diffèrent sensiblement du cliché Nord/Sud auquel on nous a habitué. Dans une large mesure, l'économie belge peut être décrite comme une structure spatiale monocentrique dont Bruxelles est le centre, mais dont les arrondissements voisins sont probablement les premiers bénéficiaires. Dès lors, il ne semble pas exagérer de dire que Flandre et Wallonie retirent de nombreux avantages de l'existence de Bruxelles, et ce quelle que soit la définition spatiale que l'on donne de cette dernière. Cette simple constatation n'est pas anodine car les forces d'agglomération discutées dans la seconde section sont souvent les mêmes qui sont à la source de la croissance économique. En revanche, elle va à l'encontre d'un sentiment largement partagé des deux côtés de la frontière linguistique, qui veut que Bruxelles soit un poids pour le reste du pays. Ce sentiment surprend toujours les observateurs étrangers tant la chose leur paraît banale que Bruxelles est un atout pour le pays. Cela dit, il devrait quand même être possible de convaincre les uns et les autres qu'une meilleure gouvernance du Grand Bruxelles va dans l'intérêt de tous. Une fragmentation politique et économique plus poussée nous ferait rentrer dans un jeu à somme négative dans lequel il n'y aurait que des perdants, quand bien même certains perdraient peut être moins que d'autres. L'enjeu est important, mais il faut reconnaître que la solution ne va pas de soi. Identifier de nouvelles formes de coopération entre entités fédérées permettant de relancer la croissance économique est indispensable pour le futur de l'économie la plus globalisée du monde qu'est la Belgique.

Analyses are performed for the complete sample, but also for sub-categories based on the country of registration underlining the spatial specificities of freight transit in Belgium. This exploratory spatial data analysis enables to reveal multi-level spatial structures associated to urban hierarchies, transport infrastructure but also firm locations or political organizations, tickling the complexity and interconnectivity of any measure taken for a more sustainable future.

La Politique Scientifique Fédérale a récemment financé (2004-2006) un projet de recherche nommé ATLAS qui a pour but principal l'exploitation des données issues du dernier «recensement» de 2001. A l'heure de la sortie des monographies et atlas, il nous semble important d'insister sur le fait qu'il s'agit desdernières images géo-statistiquescomplètes de la Belgique. En effet, à l'instar de bien des pays voisins, le recensement exhaustif de la population et des logements sera remplacé en 2011 par d'autres modes de collectes des données: enquêtes, micro-recensements et bases de données administratives. Si ces autres bases de données pourront à terme remplacer certaines statistiques produites par les recensements, de nombreuses réalités statistiques et surtout spatiales seront définitivement perdues et – surtout – ignorées. L'objectif de ce numéro deRegards économiquesn'est pas d'en faire un tour exhaustif, mais bien d'illustrer sur base d'un certain nombre d'exemples l'importance du recensement tant pour le scientifique que pour le décideur. Les exemples sont issus des domaines de recherche des trois signataires de cet article, en particulier ceux de l'économie géographique, du transport et de la santé. A titre d'illustration, voici deux exemples de réalités statistiques où le recensement procure des résultats incomparables dans tous les sens du terme: En 2001, le recensement inclut pour la première fois des questions à propos de la santé subjective des habitants, des affections chroniques, des limitations fonctionnelles de longue durée, ainsi que sur le problème des aidants naturels (c'est-à-dire des personnes prestant des soins ou des services à titre bénévole pour des malades ou personnes dépendantes). Grâce à ces données récoltées à un niveau de désagrégation spatial très fin, le recensement a permis des analysescontextuellesde la santé, c'est-à-dire d'étudier des facteurs environnementaux (telle la pollution) ou macro-sociétaux (tels le capital social ou la mixité sociale) qui, au-delà, des caractéristiques des individus, peuvent affecter l'état de santé des individus. Certes de nombreuses études contextuelles sont réalisées avec des données d'enquête (échantillon, sondage), mais leur conception ne permet pas vraiment d'appréhender les effets contextuels car le plan d'échantillonnage de ces enquêtes n'est pas conçu au départ pour une analyse spatiale. Le recensement est ici indispensable. Avec la disparition du recensement, on ne disposera plus d'information exhaustive sur les navettes (leur longueur, les modes de transport utilisés, etc.). Les «enquêtes de mobilité» peuvent compléter de façon très intéressante les recensements, mais elles ne les remplaceront jamais. En supprimant le recensement, on se prive définitivement en matière de déplacements d'une vision spatiale complète des différences locales. Or, nous savons que la situation est bien plus complexe qu'une simple «opposition Flandre-Wallonie»: certaines communes flamandes ont un profil statistique proche de communes wallonnes, et inversement! La réalité statistique belge se décline en plus de «3 moyennes régionales»; les disparités observées à l'échelle méso-géographique (communes) peuvent aussi être nuancées à l'échelle micro-géographique (quartiers et secteurs statistiques): telle est la richesse de l'analyse géographique multi-échelle, qui n'est possible qu'avec des données telles que celles issues d'un recensement. La suppression programmée du recensement décennal va ainsi nous priver d'un outil d'observation et de décision capital, en tous cas en matière de navettes (aucune autre banque de donnée ne peut remplacer le recensement), mais également dans d'autres domaines tels les logements ou la satisfaction des ménages.

This paper proposes an empirical analysis of the sensitivity of Discrete Choice Model (DCM) to the size of the spatial units used as choice set (which relates to the well-known Modifiable Areal Unit Problem). Job's location choices in Brussels (Belgium) are used as the case study. DCMs are implemented within different Land Use and Transport Interactions (LUTI) models (UrbanSim, ILUTE) to forecast jobs or household location choices. Nevertheless, no studies have assessed their sensitivity to the size of the Basic Spatial Units (BSU) in an urban context. The results show significant differences in parameter estimates between BSUs. Assuming that new jobs are distributed among the study area proportionally to the utility level predicted by the DCM for each BSU (as in a LUTI model), it is also demonstrated that the spatial distribution of these new jobs varies with the size of the BSUs. These findings mean that the scale of the BSU used in the model can influence the output of a LUTI model relying on DCM to forecast location choices of agents and, therefore, have important operational implications for land-use planning.
